Fatal accident on Construction Site

Mobile crane in position to lift steel mats to the second floor level

Bales of steel reinforcing mats were being lifted from ground level to the second floor when one bundle fell; one worker was killed and a second one seriously injured.
 
Three different parties were prosecuted for Health and Safety offences, and we were instructed on behalf of the crane operator.

Examination of the working environment revealed a catalogue of failings all the way from Management down.  Of particular significance was the common - but illegal - act of using the bale tying wires as an attachment point for connecting the lifiting chains.  The tying wires are not designed for this purpose and should not be used on site except for the placing of dunnage.
